Membership is due on the 1st July each year. Membership offers you a bi-monthly newsletter and a fortnightly venue for friendly and informative meetings. We also provide outings at public and private venues for those who wish to develop their preferred image further.

• Standard Membership - printed newsletter and $5 entry per meeting       $60 p.a.
• Standard on-line Membership - no printed newsletter, $5 entry per meeting (access to newsletter online)  $50 p.a.

Instructions

By filling out the form below and including payment (cheque made out to "The Chameleon Society of W.A. Inc.") and returning it to the address listed below, you will become a member of the Chameleon Society. This information is used to maintain the confidential contact list and the newsletter mailing list. Alternatively you can send the form electronically by clicking "Send" and then either post your cheque the usual way, transfer your payment via EFT to the Society's bank account (BSB 036 009, account number 172145, and indicate your name and type of membership), or pay our Treasurer at a Club Meeting.

How to fill in the form.

• Preferred Name The name by which you'd like to be known at the Chameleons meetings. It is also used for the contact list.

• Member Name This is the name by which we will write to or telephone you (if different from above).

• Partners Name This is the name of your wife or partner (optional).

• Email Address A valid email address if you would like to be contacted by email (optional).

• Contact Phone If you would like to be contacted by phone, put a suitable number in here.

• Postal Address A suitable mailing address, required by law. We will send your newsletter to this address if you have requested this below.

• Printed Newsletter Indicate if you would like to receive the bi-monthly newsletter by postal mail.
