By filling in the form below and submitting it with the appropriate membership fee, you will become a member of
The Chameleon Society of Western Australia. The information in this form is used only to maintain the confidential registry that we must keep by law, and for the newsletter mailing list. All information is strictly confidential.

Membership is due on the 1st July each year, and the current membership fees, together with the payment methods,
are listed on the main Society Membership page.

We are an officially Incorporated Society, and your name and full postal address is required by law. We cannot accept a membership application without these. A valid P.O. box is acceptable as a postal address. We will use the postal address only for notice of the Society AGM, extraordinary meetings, and membership renewal. If you prefer to not receive these notices, please indicate so in the area reserved for ‘Comments’.

Your ‘Preferred Name’ should be the name by which you'd like to be known at the Chameleons events and meetings.
It may also be used for the contact list if you should later opt to be included in any online contact.

Your ‘Member Name’ is the name by which we will send all correspondence to you (unless specifically requested not to correspond as mentioned above). It is also the name by which we will telephone you if you have supplied a telephone number. You may leave the ‘Member Name’ entry blank if you would like us to always use your ‘Preferred Name’. The ‘Member Name’ will not be divulged to anyone outside of the Society Committee unless you specifically give permission to do so.

Please select ‘Yes’ or ‘No’ for the ‘Printed Newsletter’. If ‘Yes’, we will send our newsletter to your postal address. If ‘No’, you will be able to download our newsletter from our website forum. Please take note of the different membership fees for posted and online newsletters.

The remainder of the items is optional. You may supply an email address if you would like to be contacted by email concerning important Society issues and events. We will also notify you of your membership status via this email address. You may supply one or more contact telephone numbers if you are acceptable to be being contacted by telephone. Telephone numbers supplied will not be divulged to anyone outside of the Society Committee unless you specifically give permission to do so.

You may specify any special considerations in the ‘Comments’ area. We would also be interested to know how you became aware of our Society and your main reason for joining, if you would like to inform us.
